Waiver Statement:
  • I understand that checking the Terms & Conditions box below, and clicking the "Continue" button acknowledges that I have read, understand, and agree to the terms and conditions outlined herein as a condition of my child/student's participation in the The Lyceum of Monterey County’s program for which I am registering him/her. I further understand that checking the Terms & Conditions box below and clicking the registration “Continue” button has the same force of law as my original signature. I understand that I am not obligated in any way to make such agreement, except as a prerequisite to my student's participation in this program.
  • Consent to Recording and Media Release: I authorize The Lyceum of Monterey County to photograph, audio, and/or video record my child during the Program; and use or distribute any pictures, audio, or video recording related to the program, camp, or activity in which my child is depicted without limitation. The Lyceum of Monterey County is granted permission to use such materials for educational fund-raising, promotional or other purposes worldwide.
  • Theft, Damage, or Loss to Child’s Personal Property: I understand that The Lyceum of Monterey County is not responsible for any theft, damage or loss to my child’s personal property while participating in the Program.
  • Adherence to Standards: I understand that my child must abide by all laws, regulations, Lyceum of Monterey County procedures, policies, and rules at all times during my child’s participation in the program. I understand that if I permit my child, or if my child chooses to participate in any conduct, excursions, or other activities in violation of The Lyceum of Monterey County procedures, policies, and rules prior, during or after the Program, which are not included or part of the scheduled Program, that they do so voluntarily and that The Lyceum of Monterey County is not responsible for my child or my child’s actions.
  • Potential Disputes Resolved in Monterey County: I agree that this Release Agreement shall be governed by the laws of the State of California, without regard to conflicts of laws principles. I agree that any dispute about the terms of this Release shall be presented to a court of competent jurisdiction in the State of California.
  • Parental/Guardian Certification: I certify that I have read (or had someone read to me) and understand this entire Release Agreement, and understand the dangers involved in participating in this Program. I am fully aware of the legal consequences of this Release Agreement, and I agree to its terms. I represent and warrant that I am eighteen (18) years of age or older and have the legal authority to execute this Release Agreement on behalf of the listed child.
  • By checking the box below and clicking the "Continue" button, I agree to indemnify and hold harmless The Lyceum of Monterey County, its affiliate programs, and its staff, trustees and sponsors, for any personal injuries or property damages arising out of my child/student's participation in the event/program for which I am currently registering. I also understand that by agreeing, I am certifying that my child/student understands what is involved in participation; that I have freely and voluntarily allowed my child/student to participate and hereby agree to assume and accept any and all risks associated with his/her participation in the event/program for which I am registering him/her.
