School Field Trip Permission and Liability Waiver

Schools bringing groups of students to the EYH STEM Conference are required to obtain a signed parent/guardian permission slip and liability waiver for each participating student. Upon registration, a waiver form will be emailed to the registering teacher or school representative for distribution and collection. By bringing a student group to the event, the teacher or school representative confirms that all required waivers have been completed and agrees to bring the signed forms to the event.

I understand that I am ranking my attendee’s workshop selections. The Lyceum will do it's best to put my child in two workshops in the top 4 choices. Workshop placement is based on several factors, one of which includes time of registration, but the Lyceum also works to equitably distribute to get all participants in at least one of their top 4 choices. Workshop placement is at the sole discretion of the Lyceum of Monterey County.
